A sports camp registration form should carry a family from “this camp fits” to a confirmed place without making them chase dates, rules, or payment instructions across separate pages.
Use the template below as a field map. It is not a waiver, medical form, or substitute for local legal, safeguarding, insurance, or governing-body requirements. Keep those requirements in the approved process for your sport and location.
Put these details before the form
The registration page should answer the basic fit questions before asking anyone to type:
- Camp name and sport.
- Who the camp is for, including age range and level.
- Every date, start time, end time, and arrival window.
- Venue, entrance, court or field, and pickup point.
- What the registration covers.
- Capacity or availability status.
- Price, payment timing, and refund or cancellation policy.
- What athletes should bring.
- What the coach provides.
Do not make the parent repeat these details inside the form. The form should collect the information needed to register the athlete and run the camp.
Copyable sports camp registration form template
Copy the sections that apply to your camp. Delete fields that do not change athlete fit, safety, payment, communication, or the staff plan.
1. Camp selection
Camp: [program name]
Dates: [all camp dates]
Time: [start, finish, and arrival window]
Location: [facility, address, entrance, court, or field]
Registration covers: [the full multi-day block / selected day / selected session]
Price and payment rule: [full payment / deposit and balance timing / free]
If the camp is sold as one multi-day progression, use one registration for the complete block. Day-by-day selection belongs on the form only when drop-in attendance is part of the offer.
2. Athlete information
Athlete full name: ______
Name used during camp: ______
Age or date of birth, if required for placement: ______
Current level or playing experience: ______
Team, school, league, or club, if relevant: ______
Camp goal or skill focus: ______
Use level questions a parent or athlete can answer. “Beginner, developing, intermediate, or competitive” is usually clearer than asking someone to interpret an internal coaching scale.
3. Parent or guardian information
For a youth camp:
Parent or guardian name: ______
Relationship to athlete: ______
Mobile number: ______
Email: ______
Best contact for camp updates: [text / email / phone]
For an adult camp, replace this section with the athlete’s contact details and add an emergency contact only when your operating policy requires one.
4. Emergency contact and pickup
Emergency contact name: ______
Relationship to athlete: ______
Emergency contact number: ______
People authorized to pick up the athlete: ______
Pickup restrictions staff need to follow: ______
Do not collect pickup information if every participant is an adult and the field serves no purpose. When pickup authorization matters, make the rule clear to both the family and the staff checking athletes out.
5. Operational notes
Allergy, accessibility, injury, medication, or other information staff need to run this camp safely: ______
Action staff should take or avoid: ______
Who may be contacted if clarification is needed: ______
Ask only for information your team is prepared and permitted to handle. A broad medical-history box can collect far more sensitive information than a small coaching business needs. Use an approved separate process when local rules, insurers, facilities, or governing bodies require medical or health records.
6. Policies and required consent
List the actual documents and acknowledgments that apply. Do not paste a generic waiver from the internet and assume it works for your business.
[ ] I reviewed the camp dates, location, arrival window, and pickup instructions.
[ ] I reviewed the cancellation, refund, missed-day, weather, and facility-closure policy.
[ ] I reviewed the participation, conduct, and equipment expectations.
[ ] I completed any separate waiver, consent, or governing-body form required for this camp.
Name of person completing registration: ______
Date: ______
Keep optional permissions separate from required camp terms. For example, photo or marketing permission should not be hidden inside a general policy acknowledgment.
7. Payment and confirmation
Before the final button, show:
- the athlete and camp being registered;
- every included date;
- the amount due now;
- any balance and when it is due;
- the cancellation or refund rule;
- the contact for registration questions.
Use a final button that describes the action, such as Register and pay or Confirm free registration.
Camp registration confirmation template
Send the useful details again after registration:
Subject: Confirmed for [camp name]
[Athlete name] is registered for [camp name] on [dates] at [location]. Arrival begins at [time] at [entrance or pickup point]. Bring [equipment, clothing, water, food, or other required items].
Payment status: [paid / deposit paid with balance due on date / free registration].
Review the camp policy here: [link]. If the athlete can no longer attend, use [approved contact or self-service action] before [cutoff].
The confirmation should be specific enough that a parent can find it the night before camp and know where to go, when to arrive, and what to bring.
Keep the form short without leaving the coach guessing
Every field should answer one operating question:
- Does this camp fit the athlete?
- Who can the coach contact?
- What does the staff need to know before the first session?
- Who can pick up a youth athlete?
- Which policies and required forms were reviewed?
- Is the registration confirmed and what is the payment status?
If a field does not change one of those answers, remove it or collect it later.
Move the confirmed registration into the roster
After the form is submitted, the coach still needs one working roster that shows who is registered, how many places remain, and what has been paid.
Elite Coach Booking treats a camp as a program rather than a stack of unrelated appointments. A fixed multi-day camp can be published as one bookable block, and the confirmed registration, payment status, roster, and check-in stay attached to that program. The programs feature shows the current workflow.
The guide to soccer camp registration software shows how to evaluate that full path from the public offer through capacity, payment, reminders, and field-side check-in.
Use the clinic and camp launch checklist to define the offer, capacity, schedule, location, and policy before opening registration. The clinics and camps solution connects that setup to the broader group-coaching workflow.
When the camp is ready to become a real booking path, build it in the Launch Room and test the full registration from a phone before sharing the link.
Sports camp registration form FAQ
What should a sports camp registration form include?
Include the selected camp, athlete details, guardian and emergency contacts for youth programs, authorized pickup when needed, operational notes, applicable policies and consent, payment, and a clear confirmation. Show dates, place, audience, level, price, and what to bring before the form begins.
Should a multi-day camp use one registration form?
Use one registration when the camp is sold as one complete block. Offer separate day selection only when drop-in attendance is part of the program design.
Should the form include medical questions?
Collect only the information your staff genuinely needs and is permitted to handle. Use an approved separate medical or health process when local rules, insurers, facilities, or governing bodies require it.
Is this template a legal waiver?
No. It is an operational field template. Waivers, releases, medical consent, safeguarding records, and similar documents should follow the requirements for your business, sport, participants, and location.
What changes for an adult sports camp?
Use the athlete’s own contact details, remove guardian and pickup fields that do not apply, and keep an emergency contact only when your operating policy requires one. The camp, payment, policy, and confirmation sections still apply.
